SERVICE COORDINATION
The Tennessee Department of Health’s Traumatic Brain Injury Trust Fund makes available a grant for service coordination throughout the state. The state is divided into service areas and each area is assigned to a Service Coordinator. The Service Coordinator's role is to work with the individual survivor and the survivor's family to assess the survivor's status and to outline a service coordination plan. The Service Coordinator collaborates and coordinates with available resources within the community on behalf of the client. .
What is our goal?
The goal of the Chattanooga Area Brain Injury Association (CABIA) is to improve the quality of life for persons with an acquired brain injury.
Who is eligible?
Brain injury survivors who live in Chattanooga and surrounding counties are eligible for CABIA services.
What services do we offer?
- support groups (including social and recreational opportunities)
- referrals to appropriate agencies
- identification of resources
- library
- advocacy and public awareness programs
- prevention information
- professional development opportunities
