Brain Injury ID Card
It is highly recommended that brain injury survivors carry with them an ID Card that helps them share with others in times of crisis how their injury affects them and quickly identifies who to call for more information or help. This also helps first-responders identify them as a brain injury. See below to fill out and print your brain injury wallet card.
